Submit Your Health Forms

You must submit the following forms: 1) personal medical history (parts I & II), 2) physician’s medical clearance form, 3) immunization form, 4) meningococcal meningitis vaccination response form, and 5) emergency action plan form. All five required forms are available here and in your applicant portal under the admitted student checklist tab. Please note, New York State requires proof of immunization from measles, mumps, and rubella before starting as a Cooper Union student.

In addition, The Cooper Union requires all students to be vaccinated against COVID-19, unless they apply for a religious or medical exemption (contact us at admissions@cooper.edu as soon as possible, do not wait until August). Your results will need to be uploaded in the CUSafe app before classes begin in the fall.
-
While completing the health forms, students are encouraged to make contact with Alex Fischer, director of student care and support, if they have specific questions about physical or mental health and/or disability accommodation services. Please email Alex Fischer at alex.fischer@cooper.edu with any questions or concerns.
